Molnupiravir 200 mg capsules, marketed under the brand name Lagevrio™, are oral antiviral medications authorized for the treatment of mild to moderate COVID-19 in adults. They are intended for patients who are at high risk of progressing to severe illness, including hospitalization or death, and for whom alternative COVID-19 treatment options are not accessible or clinically appropriate.

• Dosage Form: Each capsule contains 200 mg of molnupiravir.

• Appearance: The capsules are opaque and colored 'Swedish Orange,' measuring approximately 21.7 mm in length, with the corporate logo and “82” printed in white ink 

• Mechanism of Action: Molnupiravir is a prodrug of the synthetic nucleoside derivative N^4-hydroxycytidine. It exerts its antiviral effect by introducing copying errors during viral RNA replication, leading to an accumulation of mutations that inhibit the virus's ability to reproduce.

• Recommended Dose: 800 mg per dose, administered as four 200 mg capsules taken orally every 12 hours for 5 days.

• Timing: Treatment should commence as soon as possible after a COVID-19 diagnosis and within 5 days of symptom onset.

• Packaging: Typically, a full treatment course consists of 40 capsules, sufficient for the 5-day regimen.

• Not for Use in:

  • Individuals under 18 years of age.

  • Patients requiring hospitalization due to COVID-19.

  • Prevention of COVID-19 before or after exposure.

  • Treatment courses exceeding 5 consecutive days.

  • Pregnancy: Molnupiravir may cause harm to an unborn baby. It is not recommended for use during pregnancy unless no other treatment options are available and the potential benefits outweigh the risks.

• Breastfeeding: It is not known if molnupiravir passes into breast milk. Breastfeeding individuals should consult their healthcare provider before use.

• Common Side Effects:

  • Diarrhea.

  • Nausea.

  • Dizziness.

• Serious Side Effects:

  • Allergic reactions such as hives, rapid heartbeat, difficulty breathing or swallowing, swelling of the mouth, lips, or face, throat tightness, hoarseness, or skin rash.

  • If any of these occur, discontinue use and seek immediate medical attention.

In India, molnupiravir is available under various brand names, including Molulife (Mankind), Molena (Emcure), and Esplevir (Promomed) . It is dispensed only with a valid prescription. Patients should consult a healthcare professional to determine if molnupiravir is appropriate for their condition.
